Doctor Visit monthly till reach stable healthy weight or a realistic goal weight

-- medical physical exam and check up billed to insurance if not covered sign ABN for $100 for office visit and $40 for face to face counselling/ motivational interview/ food diary review and diet planning
Once goal weight is reached Dr Visits will be monthly ( billed to insurance or sign ABN if not covered ) and only one weight in and b12 during the weight maintenance phase. If weight gain is noted more than or equal to 10 lbs will be switched back to weight loss phase.
